GIANT OMPHALOCELE

  • Omphalocele aka Examphalos
  • Midline abdominal wall defect covered by membranes
  • Umbilical cord inserts on the membrane covering the herniated contents
  • Giant omphalocele: >5cm defect or >50% liver involvement
  • Omphalocele membrane: Inner peritoneum, middle Wharton jelly and outer amnion
  • Can get complicated in utero due to membrane rupture
  • Differentiate from physiologic gut herniation:11 to 12 weeks and doesn’t extend more than one cm
